Delivery and Pick Up times will be at my discretion
Since your dog will be living in my house with my dogs, please be considerate and make 100% sure your dogs have NO FLEAS and/or TICKS.
Please leave a slip of paper with emergency numbers, e.g., relatives, hotel or wherever you are staying with each occurrence.
Please write down any specific feeding/medication schedules.
My care fee will begin the day of arrival. A second day will be charged if pick-up is after 12 PM. (Example: You drop off your dog at 6:00 p.m. on Friday and pick it up at 1:00 p.m. Saturday…this would be considered a two-day occurrence and charged as such.)
If your dog needs grooming and I am to groom him or her, please tell me when you call for a reservation as I am usually booked two weeks in advance.
Please provide me with a photo copy of a current county dog license.
To Who It May Concern:
I authorize Barbara Burdick to care for my dog.
In the event my animal/pet needs medical attention, I hereby authorize Barbara Burdick to seek veterinary services at her discretion.
I agree to pay any and all fees incurred by either her veterinarian, listed below, or the veterinarian of her choice. I also agree to the above terms if, in the event of a personal emergency that should befall Barbara Burdick, who ever she appoints to act in good faith on her behalf, be given the same authority as I have given Barbara Burdick.
I understand that the above terms need not be reinforced with each event that Barbara Burdick is in possession of my animal/pet. That by signing below, the above becomes active with each occurrence.
